Background Checks for Unlicensed Providers
Child abuse and neglect background checks may also be referred to as Adam Walsh checks, CANTS checks, maltreatment checks, child abuse registry checks or 689 background checks.
To begin the background check process, the applicant must complete the CFS 689 Authorization For Background Check for Programs not Licensed By DCFS.
Who should fill out the 689 form?
- Illinois community mental health or developmental services agencies that are required to obtain a child abuse and neglect background check under Paul’s Law.
- Organizations required by an Illinois state agency, other than Illinois DCFS, (IDPH, IDHS) that requires a child abuse and neglect background check.
- Schools not licensed by Illinois DCFS.
- Nonprofits and other organizations that are not licensed by Illinois DCFS.
- Out of state licensed day cares and other organizations required by their state’s law to obtain a child abuse and neglect background check.
- Out of state placements, foster care, adoptions.
- Organizations that want to implement child abuse and neglect background checks for their employees, volunteers or vendors.
The form must be signed by the applicant with an ink pen to paper (handwritten signature). Electronic signatures, including signatures created using any kind of stylus, are not allowed. Fill out the “submitting agency” portion of the form (in the lower, left-hand corner) completely to ensure the results are returned to the correct requesting agency.
Once completed, submit the form by fax to 217-782-3991 OR by email to DCFS.689Background@illinois.gov.
